Content creators list a washed-out or greyish complexion, shadows beneath the eyes, chapped or parched lips, thinning hair and fragile nails, while a few share side-by-side photos from before and after treatment. Audiences are invited to check their own reflection against that checklist.

Dr Aziza Sesay, a GP in the West Midlands, explained that “ferritin face” does not amount to a clinical diagnosis, even if it can signal a problem. In her words, a pallid complexion, hair coming out in clumps or nails that break easily may go along with low iron, yet other illnesses can produce exactly the same signs.

Dr Drew Provan, a consultant haematologist, took a firmer line: reading a person’s ferritin from their facial appearance is, he said, “impossible”. He noted that someone with marked anaemia can look washed out, and that tiredness may bring on dark circles, but he called such visible clues “subjective and unreliable”.

Sesay raised another danger. Where skin is darker, shifts in colour can be harder to notice, which means a person relying on the online checklist might overlook an iron deficiency entirely.

15 µg/LFerritin below this level indicates absent iron stores, under British Society of Gastroenterology guidelines

30 µg/LFerritin below this level generally indicates low iron stores

Ferritin, haemoglobin and anaemia

The terms are often mixed up, and the difference explains why a face tells so little.

Measure

What it shows

Ferritin (a marker of how well the body’s iron reserves are stocked)

How good the body’s iron stores are

Haemoglobin

The protein in red blood cells that carries oxygen

Anaemia

The condition that follows when haemoglobin is low

The body’s iron reserves can run down well before anaemia appears. A person can therefore lack iron and feel unwell while still not being anaemic. Provan summed up the mismatch: “You’ve got no stored iron but you’re maintaining your haemoglobin. That’s the big disconnect”.

A patient’s story and a double-edged tool

In 2021, Lucy M, aged 32, who shares content on hair loss as @marvluceskin, began suffering from a racing heart, exhaustion and shedding hair. Her blood results came back as normal, and doctors diagnosed her with generalised anxiety disorder. Another test later found a ferritin level of 12 micrograms per litre, under the threshold of 15 that indicates stores are gone. She recalled that her complexion had seemed “grey” and that her hair felt lifeless and fragile, and she compared how she felt when depleted and after recovering as “night and day”.

She says social media has helped women take symptoms more seriously and given some the confidence to ask questions. She presents her content as her own experience, not medical advice.

Online posts can prompt people to seek help and to raise symptoms with a doctor.

A short video can lead viewers to diagnose themselves wrongly, which is why Sesay called social media a “double-edged sword”.

According to Dr Sue Pavord, a consultant haematologist, iron deficiency is widespread among women and frequently goes undetected. Doctors, she suggested, learn more from how a patient feels than from how that patient looks.

What doctors advise doing

Get a blood test. Provan and Sesay both stressed that a test is required, not just to confirm a lack of iron but also to find out why it occurred and to shape the treatment.

Look for the cause. Heavy periods, a diet short on iron, bleeding in the digestive tract and poor uptake of iron by the intestine may each contribute, and the treatment should tackle whatever lies behind it.

Avoid self-treating. Iron supplements taken without a diagnosis can be harmful, and self-treatment could delay investigation of an underlying problem.

Take tablets carefully. Oral iron is the usual treatment for a deficiency. Pavord advised swallowing the tablets with nothing in the stomach, preferably as the first thing in the day, and steering clear of tea or coffee at that time, since both can reduce how much iron is absorbed.

Use food too. Sources of iron include meat, beans, lentils, green leafy vegetables and cereals enriched with it.

What Lucy takes from her experience is that appearance should not be used to self-diagnose; the point is to recognise when you feel that something is wrong and to ask questions.

Photo: Andrea Piacquadio / Pexels

Videos with millions of views say dark circles and brittle hair reveal low iron, but doctors told the BBC that only a blood test can settle the question.

[30
1262,1265,1260,1264,1261,1263
post-27378","post","type-post","status-publish","format-standard","has-post-thumbnail","category-health","tag-anaemia","tag-bbc","tag-ferritin","tag-health","tag-iron-deficiency","tag-tiktok
["GET
